The Patient Centered Medical Home (PCMH) is a coordinated approach to care, focused on the patient experience.
Treatment is coordinated by the primary care physician to ensure you receive the necessary care when and where they need it, in a way you can understand.
The PCMH model seeks to strengthen the physician-patient relationship by providing care that is respectful of and responsive to your individual preferences, needs, and values, and ensuring that your values guide all clinical decisions. The physician-led care team is responsible for coordinating all of your health care needs and arranges for appropriate care with other qualified physicians and support services as needed.
